Planning and preparation are key to a smooth transfer.

Get copies of all the files and folders on
your old server and put them on the new server.

Remember to get even the hidden files such as .htaccess

If your site uses databases go in and export the old one
so you can import it into the new one.

WordPress and other CMS packages have a facility for this
in their dashboard. Other sites may require you to use
the export function in PHPMyAdmin.

Finally when you are ready, go to your domain name registration
and change your nameserver settings to point to the new host.

Keep the old site up for a month so you can change the nameservers
back to it if the new site needs more work.
